Radiology billing

Radiology Billing Services for Imaging Centers and Groups

Our radiology billing services are for radiology groups, teleradiology readers, freestanding imaging centers and IDTFs. We take over every imaging claim from the signed order to the posted payment, bill the read, the technical component or the global claim to match each site, and work every denial. Pricing is by written quote. We take 0% of patient payments. This page is for imaging practices, not patients with a radiology bill.

Who our radiology billing service is for

We bill for radiology groups reading for hospitals and outpatient centers, teleradiology practices reading for several facilities, freestanding imaging centers that own their scanners, and independent diagnostic testing facilities (IDTFs). Each bills its own mix of reads, technical and global claims, so we set your billing up by site and scanner first.

Hand your billing to us when:

  • Reads were signed but never billed, or only one component reached a claim.
  • Multi-study remittances come in below the fee schedule and nobody knows which lines are short.
  • Advanced-imaging denials for missing or expired authorizations keep climbing.
  • Your biller left, or your billing company stopped answering.
  • You are adding a site, scanner or radiologist.

What we take over and what your imaging practice keeps

We run the claim side of your imaging work for the read, the technical component or both. Our coding review against your radiologists’ documentation works from the signed report: We check every code against your clinician's documentation and never change a code without the clinician's sign-off. The figure shows how a study moves through our billing; the table splits the work between our team and yours.

Where imaging revenue leaks, and what we do about it

Imaging revenue leaks less from wrong codes than from billing the wrong component, posting against the wrong expected amount, or sending a claim the order or authorization cannot support.

What we check on your reads, in house and remote

  • Component by site. We map the scanner owner and testing site before selecting the read, technical or global claim, including remotely read studies.
  • Orders and ordering clinicians. We check the order and ordering-clinician details, then send missing information back to the referring office.
  • Mammography. We compare the screening and diagnostic reports before releasing both study lines, with the payer's required reporting checked.

What we check on your scanners’ technical and global claims

  • Imaging reductions. We post each same-session study with its adjustment reason so a normal reduction is not mistaken for a coding denial.
  • Equipment cuts. We maintain the equipment list room by room and check which equipment-related modifiers apply before release.
  • Commercial authorizations. We match the approved code, dates and authorization number to the scheduled study and flag changes to your team. Our prior authorization service files requests when your written quote includes them.

What radiology billing costs with us

Radiology billing is priced by written quote, because reading groups, imaging centers and IDTFs bill very different mixes of components. The quote uses the fee base, cap and terms on our published rate card: a percentage of insurance payments, applied the same way to the read, the technical component and the global claim, and never a share of patient payments.

Minimum$400 a month. Invoices 1–3: only percentage fees on that month's insurance payments, plus any statements or add-ons you order; the $400 minimum starts with invoice 4.
CapPercentage fees and mailed statements are capped at 9.0% of that month's insurance payments; the $400 minimum still applies from invoice 4.

How we start: moving your imaging billing to us

A radiology handover loses money first on reads signed but never billed (or billed for one component only) and on inherited denials near their appeal deadline, so near-deadline claims go first and unbilled reads follow as backlog. Your imaging practice sends us:

  1. The signed service agreement and business associate agreement (BAA).
  2. Logins for your PM system and clearinghouse, and read-only access to signed reports in your RIS.
  3. A claim-level aging export, which we sort by deadline on day one.
  4. Your site list: where you bill the read only and where you bill technical or global.
  5. An equipment list that marks any CT, film or computed radiography room.

Go-live, when your first claims go out, comes within 5 business days of access; any unbilled backlog follows within 10 business days of access. Clock starts with your signed agreement, signed BAA and working system logins; it extends day for day while we wait on access. Every inherited claim within 30 days of a filing, corrected-claim or appeal deadline is worked within 5 business days of access. Starts at access; covers up to 250 claims (more gets a day-1 deadline schedule); worked means submitted, corrected, appealed or returned with the fix.

Our billing handoff plan lists what to request from your outgoing biller, including unbilled reads. Before you decide, order The Free A/R Deadline Map: every unpaid claim over 90 days, sorted by deadline, in 5 business days. Clock starts with your claim-level aging export and signed BAA; above 10 requests in a week, we confirm your date within 1 business day.

Monthly reporting, service standards and guarantees

Your contract carries two guarantees, and each one is summarized here with its main conditions.

  • On-Our-Watch Guarantee: If a claim misses a filing, corrected-claim or appeal deadline while it is on our watch, we pay you what it would have paid. Claims we get complete 10+ business days before the deadline; contracted rate less any copay you collected, paid automatically within 30 days of final denial; capped $10,000/month; your holds excluded.
  • Clock-or-Credit Guarantee: From go-live, every new claim goes out within 1 business day of complete charge data, and every denial is worked within 2 business days of posting — or $25 comes off your invoice for each late one. Mon–Fri ET business days by your system's timestamps; worked: resubmitted, appealed or returned with the fix; auto-credited; capped at monthly fee; clearinghouse outages excluded.

We cannot make a payer pay above the imaging reduction, or pay a study done without the authorization the plan required. What we guarantee is the deadline and the clock. By the 10th of each month you get clean-claim rate, A/R days and net collection rate, each with its written definition, plus two imaging counts: studies paid at a different component than billed, and authorization denials by site. Every denied study line goes through our denial management service on the timings in this table.

We publish client results only with written permission. Until we have them, judge us by the guarantees and the free Deadline Map.

Your systems, new radiologists and data security

Your imaging billing stays in your own PM system and clearinghouse; the systems page covers how we confirm yours. A signed BAA comes before any access to patient data, and from your RIS we need read-only access to signed reports, nothing more.

When a radiologist joins your group, we can enroll them with your payers; pricing and terms for credentialing new radiologists are in the FAQ below.

Common questions

How much do your radiology billing services cost?
Pricing is by written quote. The fee is a percentage of insurance payments, with 0% of patient payments and $0 setup. The minimum and the monthly cap are in the pricing table above and on the rate card.
Can you bill both our professional reads and our technical claims?
Yes. We bill the read (26), the technical component (TC) or the global claim, site by site, based on who owns the equipment and where the patient was scanned. A reading group that also owns an imaging center gets one team for both, with the monthly report split by component.
Do we have to sign a long-term contract to outsource radiology billing?
No. Month to month. Leave with 30 days' notice and no termination fee. Want a run-out? We keep working the claims we submitted for 90 days at your plan rate. Decline it and we charge nothing on payments that post after you leave. Notice by email; optional run-out covers claims we submitted before your exit date, at your plan rate; handover file within 10 business days of exit.
How fast can you take over our imaging claims?
Go-live, when your first claims go out, comes within 5 business days of access; any unbilled backlog follows within 10 business days of access. Clock starts with your signed agreement, signed BAA and working system logins; it extends day for day while we wait on access. Inherited imaging claims near a filing or appeal deadline go first, on the sweep terms above; unbilled reads are part of the backlog once the radiologist signs them.
Can you handle teleradiology reads for several hospitals?
Yes. We keep a list of every facility you read for, bill each read with the place of service where the patient was scanned (CMS Pub. 100-04, chapter 13), and track which facility bills the technical side. Unsigned reads are flagged back to the reading radiologist so they reach a claim.
Which systems do you work in?
We work inside your existing practice-management system and clearinghouse; tell us which ones and we confirm in writing within 1 business day. For imaging we also need read access to signed reports in your RIS or reporting system. See the systems we work in.
Can you credential a new radiologist joining our group?
Yes. New-Clinician Enrollment: Clinicians who join while you're a client get enrollment filed with up to 5 payers each (the payer decides), with no per-payer fee. Further payers go through Credentialing & Payer Enrollment at $149 per payer. Billed only after the payer confirms your effective date; $0 if denied or panel closed; filed within 5 business days of your complete packet. Ready to talk? Request your radiology quote.
