Terms of use

These terms apply to anyone who uses this website, which H Holding Group, doing business as ClaimPillar Medical Billing ("ClaimPillar"), operates. The site explains medical billing rules and our services. It is general information, not legal, coding, compliance or financial advice for your practice, and billing services are provided only under a separate signed agreement. By using the site, you accept these terms.

At a glance
  • Guides summarize dated payer and government sources; check the current applicable rule before acting on a claim.
  • Calculator outputs are estimates from your inputs, and prices or guarantees bind only through a service agreement both parties sign.
  • You may link to these pages and quote short passages with attribution; the terms explain the site's use and limits.

Information on this site is not professional advice

Our guides summarize public rules from CMS, Medicare Administrative Contractors, state Medicaid programs and payers, with the sources and the date we checked them. Payer rules, fee schedules and contracts change, and the rule that applies to a claim depends on the payer, the plan, your contract and your documentation. Before you bill, appeal or write off a claim, confirm the current rule with the payer, your contract or a qualified professional.

Reading the site does not make you our client, and no page is legal advice. Our editorial policy explains how we source and date each rule and how to report an error.

Calculators and downloadable data

The calculators produce estimates from the figures you enter and the published rules they cite. They run in your browser, and a result is not a quote, a guaranteed filing deadline or a promise of payment. The downloadable timely-filing data lists each payer's limit with the source and quote we relied on; check the payer's current document before you rely on a limit. You may use the downloaded data in your own practice's work.

Prices, quotes and service terms

Prices and guarantees on this site describe our published offer. They bind us only once they are written into a service agreement that we both sign; for any work involving patient information, that agreement comes with a business associate agreement. Sending the inquiry form, an email or attending a consultation does not create a contract or book a service. Where the site and a signed agreement differ, the signed agreement governs.

What you may and may not do on the site

You may read, link to and share pages from this site, and quote short passages with a link to the page. When you use the site, you agree not to:

  1. Send patient names, health information, payer passwords or other confidential data through the inquiry form.
  2. Submit false information or another person's contact details.
  3. Disrupt the site, test its security without our written permission, or send automated form submissions.
  4. Copy the site's design or substantial parts of its content to publish elsewhere.
  5. Use the site in any way that breaks the law.

How we handle the details you send is set out in our privacy notice.

Content ownership and third-party trademarks

The text, charts, calculators and design of this site belong to H Holding Group or its licensors. Payer, software and organization names belong to their owners and appear only to identify their rules or products; naming them does not mean they endorse us or are affiliated with us.

CPT® is a registered trademark of the American Medical Association. Code descriptions on this site are our own short summaries, not AMA descriptor text. Where a page mentions a claim adjustment reason code or a remittance advice remark code, X12 publishes that code, our explanation comes from CMS or Medicare contractor publications, and the official wording is at x12.org/codes. Medicare payment amounts come from CMS's public fee schedule files.

Disclaimers and limits of liability

The site and its content are provided "as is" and "as available". To the fullest extent the law allows, we disclaim all warranties, express or implied, including warranties of accuracy, merchantability and fitness for a particular purpose. We are not liable for indirect, incidental or consequential loss, or for a denial, lost payment or penalty that results from relying on the site instead of the current payer rule. Where the law does not allow these limits, they apply to the extent it does. Our responsibility for billing services is set by the signed service agreement, not by these terms.

Changes, governing law and contact

We may update these terms by posting a new version on this page with a new date; the version posted when you use the site applies to that use. These terms are governed by the laws of the state in which H Holding Group is organized, without regard to its conflict-of-law rules. For questions, email inquiries@claimpillar.com or use our contact page.
